Our care
It’s about life! Mission Hospice helps patients with serious illness live life to the fullest. As a nonprofit organization, we put patients over profits, offering personalized, compassionate care – whether in patients’ homes, assisted living, board and care, or skilled nursing facilities. We create a personalized care plan especially for each patient, with sensitivities to cultural, spiritual, and personal preferences. Working as a team, our specially trained nurses, doctors, social workers, spiritual counselors, and volunteers help patients and families throughout the stages of a life-limiting illness, working to address physical, emotional, and spiritual needs.
Our Vision: Setting the highest standard for the delivery of care at home through unparalleled quality, compassion, equity, and individual choice
Our Mission: Delivering compassionate support and exceptional care in the home for individuals and communities, guiding them with choice and dignity to navigate significant care decisions
Our personalized care gives patients and their loved ones the opportunity to cherish their time together. Some families have shared their stories:
Our programs provide a continuum of care designed to serve patients – and their families – at every stage of a life-limiting illness, from community education and advance care planning, palliative care, Transitions, hospice, and grief support.
